I am voluntarily furnishing this information and will provide further specimens, properly collected and labeled, to Project Zenith for the purpose of DNA testing and analysis. I understand that sending a specimen to Project Zenith is an indication that I have read and agree to this Consent. I understand that Project Zenith will not process the specimen until the SMGF consent form has been signed. I understand that Project Zenith may require additional specimens for testing, and will contact me in order to obtain another specimen. I understand that there is no guaranteed turnaround time for the test results. I understand that the test results may or may not provide 100% conclusive evidence of biological enhancement. I hereby release Project Zenith, and its representatives from any and all injury, that might result from collecting specimens, and any responsibility for actions that I, or anyone else, may take after receiving the test results. I acknowledge that my personal and/or genetic information may be made available for others to view. I also agree that Project Zenith may monitor and supervise me in the case of a genetically similar matching participant within the database.
    
I also agree that my results will be contributed to the Sorenson Molecular Genealogy Foundation (SMGF) Database, a collaborative collective and searchable database and that I have read and agree to the terms in the SMGF consent form provided at:
